Hoff's disease

Hoff's disease of the knee joint is a lipomatous degeneration in his adipose tissue. In connection with the development of this pathology, serious functional disorders arise. Hoff's disease affects the pterygoid folds.

In the sub-synovial tissue, the knee joint has a large amount of adipose tissue. Its clusters are located in the form of interlayers under separate sites in the synovial membrane. Here the articular bag covers the parts of the bone, in which there is no cartilaginous tissue. Adipose tissue fills loose spaces between the bony ends and the musculoskeletal apparatus, performing the function of shock absorbers. Thus, during the movement of the joint, the tension in the synovial membrane decreases. In adipose tissue, there are a lot of lymphatic, blood vessels and nerves. These clusters are of particular importance in metabolic processes.

Goff's disease is noted more often in those people whose activities are associated with sharp numerous sit-ups, long walking. In other words, chronic traumatization of adipose tissue develops as a result of rapid extension and flexion. Often, Goff's disease affects women from twenty to forty. The risk group includes sprinters, polishers, ballet dancers.

Chronic traumatism provokes an aseptic inflammatory process in the fat folds, and repeated hemorrhages contribute to their densification. Violation of folds is often noted. As a result, the fatty tissue is replaced by a coarse connective tissue.

When carrying arthropneumograms of the knee joint against the background of oxygen introduced into its cavity, a dense sclerosized fat formation of the cellular structure is revealed.

Hoff's disease is characterized by a very characteristic clinical picture. As a rule, the patient complains of painful joint, crunching during movements. When the examination reveals effusion, extension is accompanied by the swelling of the two patella ligaments from both sides. Palpation of the joint causes soreness. In addition, there is limited movement, especially during flexion.

Therapy pathologies, as a rule, begin with the application of conservative methods. The patient is prescribed to rest for two to three weeks, immobilization with plastic or gypsum lint, baths, thermal procedures, semi-alcoholic compresses, corticosteroid preparations (Prednisolone, Hydrocortisone). Thus, puffiness is removed and blood circulation improves. The rapid therapeutic effect of injecting the drug "Hydrocortisone." Two million milliliters three times or four times every three days.

In the absence of results from the use of conservative methods, the preservation of pain, prolonged disability, swelling, as well as the detection of gross articular changes on the arthropneumogram, surgical intervention is prescribed.

During the operation, an arthrotomy is performed with the help of an internal paraptelar incision. The revision and removal of pathologically altered fatty tissues is carried out. This is also the prevention of arthrosis in the knee joint. The preservation of synovial membranes is made in such a way that they can be used to close the wound. A necessary condition is a thorough hemostasis, in the postoperative period, puncture of the joint is mandatory.

After the intervention, therapeutic gymnastics is prescribed . On the second day - exercises on the tension of the quadriceps muscle, and from the eighth to the ninth day (after the removal of the sutures) - to unbend and bend the joint, lying in bed. Until the eleventh to twelfth day, the patient moves with crutches, then with a stick. Electro stimulation of the quadriceps muscle is also prescribed.
