Medical card Ф 072 / У-04 is a kind of a pass to sanatorium-and-spa institutions. In the absence of this card or incorrect (incomplete) filling it, employees of the sanatorium (dispensary) have the right to refuse the patient in admission and treatment.

In order not to waste time in the sanatorium for the registration of medical certificates necessary for stay in the medical institution, it is necessary to allocate 2-3 days and to visit the polyclinic even before leaving for treatment. By the way, not all sanatoriums provide the opportunity to prepare medical documents on the spot.

Step-by-step instruction for a person who has received a ticket to a sanatorium

1. Visit to the attending physician-therapist. He fills the map of the above form on the basis of the patient's words and records in the outpatient card. The doctor appoints the required maps and necessary (according to the diagnosis of the patient) study.
2. Passage of all procedures and laboratory tests prescribed by the doctor (general analysis of biological fluids, feces on eggs, response of fluorography, electrocardiograms, other procedures).
3. Additional consultations of narrow specialists, if required by the diagnosis (for women, mandatory conclusion of a gynecologist).
4. The dermatologist - makes a note in the card about the absence of pediculosis and scabies (all mandatory).
5. A note by the epidemiologist about the absence of contact with infectious diseases during the last three weeks at the place of residence (all mandatory).
6. Re-visit the therapist to receive the form of the sanatorium card that he completed.

In the certificate for the sanatorium there must be a record:

• Epidemiologist (not earlier than three days before leaving for a sanatorium)
• Dermatologist
• Gynecologist (women)
• Answers to blood, urine and feces
• Statement of preventive vaccinations
• Records of the therapist about the disease with the indication of the diagnosis (according to the ICD-10 code), the presence of contraindications to certain types of sanatorium treatment, the available allergic reactions in the patient and the last treatment in the sanatorium, if it was passed.
• Records of other specialists and the results of additional studies - if necessary.

It is also necessary to check whether the passport data and insurance policy number are properly filled in the corresponding columns of the sanatorium card, the presence of necessary seals and signatures of doctors.

The return card in the card Ф 072 / У-04 is filled in by the health workers of the sanatorium (dispensary), given to the patient on hand at the end of the treatment and then sutured to an outpatient card for examination by the treating doctor.
