Zoledronic acid: pharmacological action and analogues

It is known that bisphosphonates have the property to suppress the development of bone metastasis and prevent the loss of bone tissue. One of the best drugs belonging to this group is zoledronic acid. There is also evidence that this drug has inhibition of antigenogenesis, tumor invasion, adherence of tumor cells in bone tissue, induces apoptosis, and synergism with chemotherapy is observed.

Zoledronic acid monohydrate - 4.26 mg, excipients - sodium citrate, D-mannitol.

Pharmacology

Inhibitor of bone resorption. Zoledronic acid belongs to a new class of bisphosphonates of high class. It has a selective effect on bone tissue. Reduces the activity of osteoclasts, without having a negative impact on the formation and other qualities of bone tissue. The effect of bisphosphonates on the bone is based on a high similarity to the mineralized bone tissue, but the exact mechanism of the molecules that provides the inhibitory activity of osteoclasts has not been studied to this day. It also has a direct antitumor effect, which provides efficacy in bone metastasis.

In vitro, it has been found that zoledronic acid, inducing apoptosis and suppressing cell proliferation, has an antitumor effect directly on the myeloma, as well as breast cancer cells, while reducing the risk of metastasis. Also zoledronic acid has the property to reduce the concentration of calcium in the serum with hypercalcemia, which is caused by the tumor.

Indications for use

  • Osteolytic foci in multiple foci of myeloma.
  • Osteoblastic, osteolytic, mixed bone metastases of large tumors.
  • Hypercalcemia, which is caused by malignant formation.

Contraindications

  • Pregnancy and breastfeeding.
  • Renal insufficiency.
  • Children and adolescents (safety of use not established).
  • Hypersensitivity to bisphosphonates and zoledronic acid.
  • With caution: impaired renal function, bronchial asthma, hepatic insufficiency.

Side effects

Anemia, thrombocytopenia, headache, dizziness, conjunctivitis, nausea, anorexia, vomiting, diarrhea, bone pain, arthralgia, hypophosphatemia, hypocalcemia, fever, flu syndrome, asthenia, weight gain. In patients with zoledronic acid, osteonecrosis of the jaw is sometimes observed (usually after surgical dental intervention).

Mode of application

Intravenous and drip for 15 minutes, but not less. In metastases of multiple myeloma , a dose of 4 mg once a month is recommended. With hypercalcemia, which is caused by a malignant tumor, the dose is 4 mg once.

special instructions

After the drug is introduced, it is necessary to control the concentration of magnesium, phosphorus, creatinine and calcium in the blood serum. If possible, dental procedures should be avoided.
