Multiple organ failure as a stress-reaction of the organism

The term "multi-organ failure" was first formulated in 1973 in a work devoted to the rupture of the aneurysm of the abdominal aorta. A little later the concept was clarified by A.Baue and D.Fry. They finally established, somewhat expanded and classified the symptoms, indicative of this grave disease.

To date, the term "multi-organ failure" means an extremely serious pathological condition that develops as a reaction to surgical intervention, sepsis, purulent diseases. In addition, the cause of the disease can be eclampsia, diabetes, meningoencephalitis, poisoning.

The syndrome of multiple organ failure can be provoked:

  • Acute or profuse blood loss.
  • Heavy shock.
  • Injuries to the skull.
  • Contusions or damage to the heart.
  • Hemopneumovarax.
  • Multiple fractures.

The pathology that arises in the body as a kind of stress reaction affects two or more body systems responsible for normal vital activity.

An example is the violation of the general gas exchange in the body, which usually develops on the second day of the post-traumatic period and is almost always accompanied by acute renal or hepatic insufficiency.

Most susceptible to the development of a condition called "multi-organ failure", smokers, diabetics, drug addicts, people abusing steroids and cytostatics.

Paradoxically, the appearance of the disease is due to the success and rapid development of resuscitation.

Earlier, when the resuscitation was just getting to the feet, most patients died from shock or acute blood loss.

Today, medicine quite successfully and quickly copes with the shock state. For example, in case of a loss of blood, jet infusions (infusions) are used. In response to this on the 2-4 day in the body of the victim develops multi-organ failure, affecting several organs or systems.

Deviation can develop in one stage or gradually.

Single-phase PON is characterized first by a violation of gas exchange, which is later joined by a developing failure of cardiac activity, liver, kidneys, lungs, and other organs. In this case PON is the last complication, after which the patient's death follows.

In the two-phase course of the disease, the short stabilization of the patient withdrawn from the shock state is disturbed by sepsis, leading to PON and death.

Doctors have established the stage of development of multiple organ failure.

1. Violation of gas exchange, blood clotting, a decrease in platelets, but an increase in bilirubin and some other enzymes.

Later, to the already existing disorders, an infection joins, due to which the kinin system is activated, neurohumoral shifts appear, blood circulation is disrupted. The insufficiency of many organs develops, stress ulcers of the intestine appear.

2. Decompensation or irreversible changes occurring at the subcellular level.

PON is better not to treat, but to prevent, conducting active resuscitation, aimed, including, and on the occurrence of a stress reaction.
