Drug 'Prestans', instructions for use, the composition of the drug

The drug "Prestans" is a combined antihypertensive drug that performs in the body a dual role:

- calcium channel blocker (slow) - BCCC;

- ACE inhibitor (angiotensin-converting enzyme);

The trade name of the drug is Prestan. Its international non-proprietary name reflects the composition of the drug - perindopril arginine + amlodipine. The drug "Prestans" is released in tablets, the instruction indicates different combinations within the dosage form, on this basis, there are 4 different forms of the drug:

- 5 + 5, that is, 5 mg of perindopril arginine, 5 mg of amlodipine;

- 10 + 10 (composition according to figures);

- 5 + 10;

- 10 + 5.

In order to understand the mechanism of work of the drug "Prestans", the instruction recommends that you first familiarize yourself with the pharmacological properties of each component separately, and only then study the effect of the agent in the complex.

The enzyme inhibitor - perindopril - is able to convert angiotensin first to the second (ACE inhibitor). It has a therapeutic effect due to its active metabolite. Perindopril is used in the treatment of hypertension, both mild and severe, while minimizing peripheral vascular resistance, as a consequence, there is a decrease in blood pressure. Perindopril is able to improve peripheral blood flow, while keeping the heart rate constant - the heart rate. Antihypertensive properties of the drug are able to manifest themselves after a maximum of 6 hours. Absorption of the drug occurs quickly, within an hour the plasma reaches its highest concentration, due to this speed of exposure, a rapid decrease in blood pressure occurs. For the beneficial effects of the drug, perindopril must be converted into perindoprilat, if taken with food, then this process slows down. Therefore, a one-time reception is recommended every 24 hours in the morning before meals. The body is excreted by the kidneys.

Amlodipine - the second component of the drug "Prestans", the instruction calls it a dihydropyride derivative, BCCC. The action of amlodipine, as an antihypertensive drug, is based on direct action on the vascular walls, namely, on smooth muscle cells. Expanded arterioles reduce afterloading on peripheral vessels, without changing the heart rate and reducing the need for oxygen in the myocardium. The antihypertensive properties of amlodipine develop slowly, in view of some characteristics of the drug, it can be taken in patients with bronchial asthma, gout, diabetes mellitus. After receiving amlodipine, its maximum concentration is achieved only after 12 hours, while it does not matter when the drug is taken with respect to food intake, it is excreted by the kidneys.

In the ASCOT-BPLA studies, the effectiveness of long-term combined use of perindopril and amlodipine was studied, with patients with hypertension aged 40 to 79 years taking part. The main criterion for evaluation was the number of non-fatal outcomes of myocardial infarction. The result showed in the patients of this group the incidence of such diseases is 10% less.

Among the indications for the use of the drug "Prestans" the instruction indicates arterial hypertension, accompanied by (or not accompanied by) ischemic heart disease.

From the contraindications to the use of the drug "Prestans" the instruction notes:

- high sensitivity to the components of the drug;

- age to 18 years: contraindications are based on insufficient study of the effect of the drug on a young organism;

- kidney failure;

Lactose intolerance (hereditary).

The drug "Prestan" instruction prohibits taking during pregnancy and lactation. Information on treatment in case of an overdose is not available.
