New teeth for 3 days - basal implantation of a new generation

Each of us wants to have a beautiful smile. But, unfortunately, inadequate oral hygiene, poor ecology, eating disorders and simply age-related changes lead to the fact that sooner or later we lose our teeth. Solving the problem of lack of 1 or a pair of teeth is quite simple - there are many options for patients. But if there is not enough of the majority or even all of the teeth, or some of them are already in an extremely unsatisfactory state, alas, there are not so many solutions. These are only removable dentures and dental implants.

With removable dentures, everything is clear - these are designs that are fixed only on the gums. Often drop out, strongly rub mucous, lead to acute discomfort. Everyone who wore removable dentures knows that at a low price there are many problems.

Many people also heard about implantation. And if someone considered this option, he knows that with a long absence of teeth and a prolonged wearing of removable dentures, bone tissue atrophies, that is, shrinks in volume. And to install the implants, you will have to perform an additional procedure - bone plastic. Unfortunately, this noticeably stretches the process of treatment (sometimes up to 1.5-2 years!), And the cost due to add. Operations increases.

But classical implantation today has a rather successful alternative - a basal implantation. According to statistics, in 97% of cases it is possible to install implants without bone plastic. And the duration of treatment from the moment of installing the implants to fixing the prosthesis (by the way, non-removable) is only 3 days. Let's look at whether this is possible and whether there are any pitfalls.

Briefly about important features of the method

The method of basal implantation is one-stage implantation with immediate loading. That is, implants are installed almost in one visit, and after a couple of days they are fixed on prostheses. And non-removable and virtually permanent. This is one of the main differences of the method - the patient returns to a full life without tedious waiting and wearing removable dentures during the intergrowth of implants and jawbone cells. The prosthesis itself plays a very important role - since the method is indicated with bone atrophy, and its volume is too small for fixation, the prostheses are combined into a single design of the implants and thus fix them in a stationary state.

The second important difference of the method is in the implant models and the technology of their installation:

  • Single-piece models are used (the part that is located in the bone is connected to the apex, called the abutment). They are established by a method of a puncture, that is are screwed in a bone without exfoliation of a gum and bone drilling. Without tissue incisions,
  • Implants can be installed under a slope, which is why the method is relevant to use in the presence of atrophied bone, because the angular arrangement allows you to bypass the most unfavorable areas where the volume is not enough. That is, the indications for using this technique are the lack of a large number of teeth (usually all in the jaw - that was the place where to put the implants, but at least three in a row), as well as a violation of the jaw bone volume,
  • Implants are installed in the basal (deep) bone layers. Sometimes even elongated models are used, which involve even deeper sections - the joint bone and buttresses or the lines of force of our skull. The density of such layers is much higher than the central alveolar part, into which the implants are implanted. This is also one of the reasons why basal models are fixed better,
  • Prosthetics is carried out immediately. Prostheses have a metal base, which allows you to connect the implants and fix their position. In addition, with the updated teeth, the patient gradually begins to chew food - the pressure is transferred to the bone, which begins to function in a completely natural way. That is, it is quickly restored and grows around implants. Therefore, the goal of immediate prosthetics is not only in aesthetics.

A great advantage of the method is the shortened treatment time - the whole process takes only one step. A comfortable and comfortable prosthesis completely restores the aesthetics of a smile and returns the opportunity to chew food. That is, the duration of treatment is markedly reduced, which allows the patient not to limit himself practically in anything and in a very short time to return to a full rhythm of life.

How the treatment works: stages of work

1. Diagnosis, preparation of the patient: the delivery of tests, computed tomography to ensure that the doctor was able to assess the quality and structure of bone tissue, to exclude contraindications. Basal implantation "works" in conditions of atrophied bone, therefore it is important for the doctor to estimate the volume of tissue from all sides (this is exactly what 3D tomography allows). You may even need to donate blood for analysis - if the doctor has doubts about the patient's health. If there are any diseases that become an obstacle to implantation, the patient is sent to the specialized specialists for consultation and appropriate treatment.

2. 3D modeling: the entire treatment process is carefully planned. But not "on paper", but with the help of computer software. The CT data are loaded into the computer, the program independently calculates the volumes, selects the most optimal places for the implants, and even conducts their virtual implantation. Naturally, this process is carefully controlled by the attending physician.

3. Immediate installation of implants : after the 3D visualization individual templates are developed - they are used in the surgical stage. These are the mouthguards on the dentition, inside there are holes - through them the implants are inserted into the bone.

Moreover, the surgical stage is slightly light compared with the classical approach - the implants are screwed into the bone, while the gum does not exfoliate, the seams after also are not superimposed. In addition, the method allows the installation of implants simultaneously with the removal of sick teeth - the "screws" are put in the same hole, at the same time.

4. Prosthetics: after the implants are installed, the angle of inclination of the apex or abutment is corrected. After all, it should be positioned in such a way that it is possible to fix the prosthesis smoothly, in accordance with the existing bite or even even it. Immediately, the impression is removed - it is transferred to the laboratory, where the dental technician creates the prosthesis. It is fixed for 2-3 days.

For the time of implant engraftment, an adaptive fixed prosthesis is fixed on them. It has a metal base, the purpose of which is to connect the installed "screws". It also provides for a plastic gum that will hide the uneven shape of its own mucosa (it changes due to a prolonged absence of teeth). And the third part of the prosthesis is crowns. They are made of plastic, but with the addition of diamond crumb to strengthen the material and improve its aesthetic characteristics. The very first prosthesis should be worn for at least six months. However, implantologists note that it is designed for 3-5 years, so there is no urgent need to remove it: for cermets or zirconia it can be replaced after the end of life.

A lot of controversy goes around prosthetics with basal implantation. Pros and cons are expressed by orthopedists and implantologists. However, in fact, the patient does not have to worry about the aesthetics of the teeth: only the maxillofacial surgeons with orthopedic knowledge are allowed to conduct the basal technique, since it is not enough to install a dental implant, it is important to secure the prosthesis so that it is beautiful and comfortable. This also helps and computer modeling, which allows you to take into account all the nuances.

What are the advantages of the method in comparison with two-stage implantation?

Basal implantation

Classical two-stage implantation

No bone loss is required even with acute atrophy

If there is a lack of bone volume,

Possible prosthetics for inflammation of the gums and bone tissue

In inflammation, implantation is carried out with caution or not at all

Even a large number of implants are installed all at once

Often requires fractional installation of implants for prosthetics of the entire series - due to traumatic surgery

If you want to remove live teeth, implants can be installed simultaneously, including in the same hole

After the removal of the teeth, there must be time for complete rehabilitation of the bone tissue (about 3-4 months).

Rehabilitation after the installation of implants takes several days - their fixation is by puncturing

Rehabilitation lasts for several weeks - the patient needs to carefully monitor the condition of the oral cavity and superimposed sutures

For the time of implant engraftment, a permanent prosthesis is installed, it can be replaced at least six months to a year, a maximum of five years

For the time of implant engraftment, a removable prosthesis is fixed, its replacement with a permanent structure is carried out after 4-6 months.

After prosthetics in the course of the month, the patient completely returns to the usual diet

A patient can fully eat only after installing a permanent prosthesis, removable require compliance with the diet

Term of treatment - not more than a week

The duration of treatment is at least 4-5 months, with bone growth - about a year

In general, basal implantation is a more promising method, especially when multiple restorations are required. After all, the patient gets new teeth at once, he can fully smile, enjoy his reflection in the mirror, do not limit himself to nutrition. However, the classical approach does not need to be discarded - it allows you to achieve an ideal aesthetics (in terms of the gum position), but it is better to apply it in single restorations, because when you restore all the teeth from treatment, you simply get tired.

As for the minuses of basal technology, they are unquestionably available. But they are not reduced to technological features of the methodology, but rather to the lack of experience of many doctors in the application of this approach. When choosing a clinic for prosthetics, be sure to pay attention to the admissions to the work of doctors - today the basal technique is the right to conduct specialists who have been trained in the international fund of implantologists who certifies doctors with specialization in this direction. It will be a plus if the doctor is a member of other public dental organizations - this further confirms the qualification and seriousness of the approach to treatment.

Important! The implantation procedure itself should be performed by the doctor maxillofacial surgeon, with excellent knowledge of the anatomy of the jaw system, because he has to work in a very limited space - with a lack of bone volume.

Cost of treatment

Treatment according to the classical two-stage method is more expensive. The price of basal implantation, as a rule, is fixed, that is, it is a turnkey implantation, since the doctor initially knows all the manipulations and additional procedures are not required. The cost of restoring one jaw is about 230-250 thousand rubles (almost two times less than classical implantation). This includes preparation, anesthesia, implant placement, and prosthetics. Savings are explained by a noticeable reduction in the stages of treatment.

What are the alternatives?

When prosthetics of the full dentition, the patient, in addition to the basal technique, can consider two more implantation options: all-on-4 (it is called so because only 4 implants are used to support the entire prosthesis) or all-on-6 , Six pieces of artificial roots are used). But if the basal method is used precisely for acute atrophy, the first method mentioned is with sufficient bone volume or slight shrinkage, and the second - with medium-level problems. Therefore, which option is suitable for a particular patient, only the attending physician should determine.

On a note

Today, basal implantation, as mentioned above, can not be done anywhere. Below we will present several clinics, whose doctors have valid international certificates confirming the right to work on the method of basal implantation:

  • Center for Dentistry "Stars Dental" on the Barrikadnaya metro station, www.starsdent.ru
  • Dentistry Smile-at-Once, www.smile-at-once.ru

In both dentists there are special programs for the organization of treatment for patients from other parts of Russia and countries of the neighboring countries.
