Filling the T2 card (sample). Employee's personal card

Form T2, or Personal Card, is included in the list of personnel documents required to conduct in any organization. Information on employees, both current and dismissed, periods of work, information on professional qualifications and work activities - all this should contain a correctly completed unified form of T2. The employee's personal card is a personnel document of a legally approved form and should be entered into immediately after the order for the employee's acceptance has been agreed and signed. How to correctly enter information in this document? What information should a personal T2 card contain?

Basic requirements and features of filling

A personal card is filled in for each employee, including part-time employees, in one copy. The card T2 is obligatory for filling in the organizations, however for the workers borrowed from the individual businessman, this document can be conducted only at the request of the employer. Other exceptions are civil servants, documentation of a different form has been established for them.

The filling of the T2 card, a sample of which is presented in the article, is carried out in accordance with the mandatory requirements fixed by law:

• All information is entered into the established sections, legibly and without abbreviations.

• Abbreviations are allowed when specifying the place of residence or information about education, but in a generally accepted form that does not distort the meaning of the information.

• The completed form T2 should contain codes corresponding to the all-Russian classifiers: OKIN, OKATO, OKSO, OKPDTR. The number designation must be entered in a special table on the right side of the line. All code marks are freely available in printed reference books, as well as on the site "All-Russian Classifiers".

• If the classification code is incorrectly filled in, the T2 form is considered invalid, in this case it is required to create a new personal card. Strikethrough, corrections in the coding table are not allowed.

• Changes in the employee's work or personal data should be reflected in time in T2. All changes made to the personal card are certified by the signature of the HR specialist.

• Information is entered in T2 on the basis of supporting documents provided by the employee.

• Words and phrases "no", "was not (a)", "not available", as well as dashes in the form T2 are not entered - such lines are left blank.

Grounds for entering information about employees

The personal T2 card must be completed on the basis of the following documents:

• an employment contract with the employee;

• Orders for organization: on employment, transfer, granting leave, dismissal;

• a passport (another document replacing it) of the employee;

• SNILS;

• documents confirming the education received, passing courses, attestation sheets and so on;

• Certificate of INN;

•military ID.

Other information (for example, the degree of proficiency in foreign languages) is indicated only with the words of the employee. If necessary, the employer can be requested other supporting documents, in accordance with the specifics of the professional orientation of the organization, as well as the established internal regulations.

How do I fill in the title?

The first page of the form contains information about the employee and the level of his professional qualification. Filling in of the T2 card (sample submission is shown below) starts with the name of the employer's organization on the top line of the form. The name is indicated in full, in accordance with the organization's statutory documents. In the coding table the digital designations OKPO and OKUD are put down. Further, the following data is entered in the table below:

• Date of filling in T2 (Arabic numerals);

• Personnel number assigned to the employee during employment;

• TIN (based on the certificate of the Federal Tax Service);

• SNILS employee;

• the first letter of the employee's surname is placed in the alphabet column;

• the nature of the work, in accordance with the terms of the employment contract: temporary, permanent;

• type of work: can be either "basic", or for external or internal part-time employees - "concurrently";

• The floor is indicated by the letters "F" or "M".

After processing the title of the document, you can go to other sections, which include the initial filling of the T2 card. Sample of entering information in the title:

General information about the employee

Completion of this section begins with the indication in the table "Labor contract" of the number and date of the employment contract concluded with the employee. Further, the filling of the T2 card, the sample of which is shown below, is carried out in the following order:

1. In the line "1" the full name, as well as the name and patronymic of the employee, are fully indicated.

2. In the line "2" the date of birth is indicated in text form. Example: "April 20, 1981". In the table, the same date is coded in Arabic numerals: "20.04.81".

3. Line "3" should contain information about the place of birth. Here it is important to be guided by the following requirements:

- When specifying a settlement with the words "city", "village", "region", "village", "district", "edge", generally accepted abbreviations are accepted: "obl.", "Cr." And so on. Example: "mountains. Samara "," pos. The taiga. " However, the designations "village", "kishlak", "village", "aul" are recommended to be indicated completely.

- The total number of characters in line "3" must not exceed 100 characters.

- The encoding table is filled in accordance with the All-Russian Classification of Territorial Entities (since 2014 - the new name is OCTMO code).

4. Line "4" is intended for entering information about the citizenship of an employee on the basis of a passport. All subsequent information in this section is made in strict accordance with the code of the OKIN. Enter information as follows:

• citizen of the Russian Federation (the code "1" is entered in the encoding table);

• code "2" is indicated if the employee has dual citizenship, one of which is Russian citizenship; A note about it looks like this: "A citizen of the Russian Federation and a foreign state";

• for employees who are citizens of another state, in the encoding table the code "3" is indicated, in the line "4" the entry is entered: "Foreign citizen";

• in case the employee is a stateless person, the "Person without citizenship" is entered in the line, in the coding table - "4";

5. The line "5" indicates the knowledge of the foreign language worker and the level of language proficiency. The encoding of the OCIN assumes three categories characterizing the level of possession of a letter and a spoken language in a foreign language:

• code "1" - translates and reads with the dictionary;

• code "2" - reads and can be explained;

• code "3" - freely owns.

In addition, the OCIN contains specific code symbols for foreign languages, which must also be indicated in the coding table. Thus, the information in line "5" is entered in the following order: the name of the foreign language - the level of possession - the code name of the language and the degree of possession of it (in the table). Filling of the T2 card, sample: "German - I speak fluently". The code according to the OKIN: 171/3.

6. The sixth line of the section "I" is filled only on the basis of the education documents provided by the employee. An example of the text designation and the corresponding OKIN codes are presented in this table:

Further in the table below you should indicate: the name, series and number of the education certificate provided by the employee, the year of graduation, as well as the qualifications received and the name of the profession in strict accordance with the document. The code of the profession is entered in the table. If the employee is provided with two documents on education, for example, secondary vocational and higher or first and second highest, in T2 information on all available documents is entered. Similarly, sections IV-VI include information on retraining, attestation, and advanced training received by the employee after employment.

Information on unfinished education should also be entered in T2, indicating the type of education available. For example, for an employee who is studying at the time of employment, information should be provided on the education that was available before entering the current educational institution; On the basis of a certificate from the school, indicate that the employee is currently studying, the name of the institution and the course of study. Similarly, information is entered if the employee was trained, but did not finish his studies, but expelled (or was expelled):

• with less than three courses completed, only the education that the employee had before the entry should be indicated;

• Information on the training completed, subject to the completion of three or more courses of the educational institution, is entered in T2 on the basis of a diploma (certificate) on incomplete higher education; In this case the designation "Incomplete higher education" and the rate from which the employee was expelled are indicated;

7. In lines "7" the full name of the main and, if available, additional position (profession) of the employee, which he occupies in the organization according to the order on employment and the staff schedule, is indicated. Abbreviations or distortions of the name are not allowed. In the section coding table the code of the position according to the OKPDTR is indicated.

Work experience

It is made in accordance with the previous work activity of the employee, the basis is the work book. In case the employee is employed for the first time, the first line of the "common" is marked with zeros.

Information about the employee's relatives: who should be indicated?

Completion of this section begins with the indication of the marital status of the employee at the time of employment. It is necessary to avoid words and phraseologisms "married", "in a civil marriage", "cohabitates" and so on: the line is filled with the wording defined by the all-Russian classifier with the code of the OKIN.
When changing the marital status, the amendments are made on the basis of the certificate of the registry office, indicating the requisites of the document.

Information about relatives directly living with the employee, indicating the degree of their relationship, should also be entered into such a document as a personal card of the employee. The sample of filling T2 is as follows:

Military records

Information on the suitability for military service is filled on the basis of the military ticket provided, indicating the rank and category of the stock marked in the document. Information for points 3-4 is on page 3 of the military ticket. For an employee who has not yet served, but who does not have any restrictions for the service, the lines are left blank, with the entry "on line" in line 2.

The degree of eligibility for the passage of service is indicated on the basis of the registration certificate by the letter designation:

• Year - A. This category is also displayed if there is no indication of the category in the documents.

• Suitable with restrictions - B.

• Limited in use - B.

• Temporarily unfit - G.

The line 8 of the section is intended for the marks "Removed from the military register", indicating the reason, for example, by age or state of health.

Page 3, section III

This section specifies only information about the transfer of an employee to another structural unit within the same organization (including transfer to another locality) or about changing job functions. All information is entered on the basis of orders for the organization with the indication of the requisite of the order (date, number).

Important to remember! It is not necessary to indicate information about changes in the rate or salary that have been carried out without changing the position of the employee.

With all entries made in this section, the employee must be acquainted with the signature.

Holidays and rest days of the employee

This section is filled in as the employee is given leave in accordance with labor law and internal regulations. In the table of the section, it is necessary to enter the details of the vacation order, the type of leave, the number of calendar days. For the annual leave, you should also indicate the period for which vacation days are given, in columns No. 2, 3 of Table VIII (employee's personal card). The sample of filling is as follows:

Section X: What information can I specify?

"Additional information" is a section provided for information that may be relevant to work activity, but not specified in other sections of Form T2. Such information can include, for example, the employee's driving experience and driver's license of a certain category, health restrictions (in accordance with medical opinion), and so on.

How to correctly enter information about the termination

The last section is completed on the last day of the employee's work. It is necessary to specify the requisites of the order, the reason for the termination of the employee and to certify the made entry with the signature of a regular employee. With the records of section XI, the employee must also be familiar with the signature.
